Create an app interface with controls
(1) recording a conversation between patient and doctor. create a wav file.
(2) button for start record and stop recording
(3) On stop recording, call an API with wav file, show recognized text
(4) UI with fields for

• Name
• Age
• Gender
• Chief complaint(s) (symptoms and duration)
• History of present illness
• Past medical history - including Surgical history, Family history, Social history (smoking, alcohol, occupation, lifestyle)
• Allergies
• Examination Findings - including General examination (vitals: BP, pulse, temp, respiration, SpO₂, weight, height, BMI), Systemic examination (cardiac, respiratory, abdominal, neurological, etc.), Local examination (if applicable)
• Provisional Diagnosis
• Investigations - including Lab tests advised, scans
• Treatment Plan - including Medications prescribed (name, dosage, frequency, duration), Procedures advised (if any), Lifestyle/dietary advice
• Follow-up - including Review Date, Referral to another department
