Report to include

Patient Name/Age/Gender | UHID | Visit DateTime | DR Diagnosis (Automated) | DR Diagnosis (Manual)

The DR Diagnosis (Automated) | DR Diagnosis (Manual) need not have values for patients whose manual assessment not available.
